  1. Based from the 2020 Accomplishment Report of the Division of Science City of Munoz conducted on January 12, 2021 at the DepEd Regional Office, there is a need to monitor result of the assessment of learners and the effectiveness of the assessment tool used so as to ensure that no learner is left
  2. In line with this, the following schools attached hereto shall submit Intervention Program Monitoring Tool and Accomplishment Report of Intervention conducted for Q3 of School Year 2019-2020.
  3. Moreover, to monitor Learners’ Performance for Q l of School Year 2020-2021, all schools are required to submit result of learners’ performance based from the Mastery Tests conducted by grade level per learning area using the template  to be
  4. All reports needed shall be uploaded in the Google Drive “2021 School Reports”.
  5. Submission of reports needed shall be on Tuesday, January 19 at 10:00 AM for
  6. Attached is the list of schools to submit Intervention Program and Accomplishment
  7. Immediate dissemination of this Memorandum to all concerned is desired.
